Pen writes without touching the screen

Anyone facing this issue?
Hovering the pen just above the screen would "sometimes" gets detected as I'm writing on it.
Removed my screen protector and tried different apps but still the same.

I can scroll and draw or write with the pen just hovering.
I can't write my notes because of this.
